EU-KMU and de minimis declaration form completed and signed by the applicant

Consultancy report signed by the consultant and the applicant (including questionnaire on the cross-sectoral principles of the ESF)
Invoice from the counselling company
Account statement of the applicant as proof of full payment of the fee
Confirmation letter from the regional contact person (only for companies up to one year after foundation at the time of application)
